Important Instructions:
Step 1
Order and pay for your blood tests online using this convenient blood
testing order page.
Step 2
Download and print
the Customer Blood Test Authorization Form. Fill out the form
completely. If you ordered more than 5 tests, fill out and sign
additional forms as needed and be sure to sign each form.
Mail the signed form(s) to:

Step 3
After your authorization forms have been received, you will be mailed
either a requisition form to take to your local LabCorp Patient Service
Center or a Blood Draw Kit, whichever is applicable. Please note: if a
blood draw kit is used, an additional local draw fee may be incurred.)
Step 4
Please do not eat anything for 12 hours prior to having your blood
drawn. Most people have their blood drawn in the morning before eating
breakfast. To avoid dehydration, you can and should drink water before
having your blood drawn.
Step 5
After you visit your local blood-drawing station and have your blood
drawn, your blood test results will be mailed directly to you. |

Glucose,
fasting: This test directly measures glucose levels and is
commonly used in the evaluation of diabetes.

Uric acid:
This test is used in the evaluation of gout or recurrent urinary
calculi.

BUN (blood
urea nitrogen): This test is used to measure liver function
and indirectly to assess renal function and glomerular filtration
rate.

Creatinine:
This is a renal function test used to estimate glomerular filtration
rate, and follow progression of renal disease.

BUN/Creatinine
ratio: This test is used to diagnose impaired renal
function. With creatinine, BUN is used to monitor individuals on
dialysis.

Sodium:
This routine test is used to evaluate and monitor fluid and
electrolyte balance and therapy.

Potassium:
This routine test is used to evaluate and monitor electrolyte
balance and is especially important for cardiac individuals.

Chloride:
This test by itself does not provide adequate information. However,
as part of a multiphasic testing for electrolytes it can give an
indication of acid-base balance and hydrational status.

Calcium:
This test is used to evaluate parathyroid function and calcium
metabolism.

Phosphorus: This test is used to measure serum phosphorus.
An imbalance could indicate the possibility of any number of
conditions.

Protein/Albumin/Globulin: This test is used to assist in
the detection of many diseases that affect blood proteins as a whole
or one single fraction of protein.

Albumin/Globulin Ratio: This test is used to evaluate renal
disease and other chronic disease.

Bilirubin:
This test is used to evaluate liver function

Alkaline
Phosphatase: This test is used to detect and monitor liver
and/or bone disease.

LDH
(Lactic dehydrogenase): This test measures the
intracellular enzyme LDH which, when present in the blood, can
support the detection of injury or disease.

AST (SGOT):
This test is used to evaluate the possibility of coronary occlusive
heart disease or liver disease.

This test is used to identify liver disease and to distinguish
between the liver and red blood cell hemolysis as the source of

Homocysteine has been shown to be an
independent risk factor for the premature development of coronary artery
disease and thrombosis. This test is intended for use in screening
individuals who may be at risk for heart disease and stroke. Studies
have shown that even moderate levels of homocysteine pose an increased
risk for arteriosclerosis compared with the lowest 20th percentile (<7.2
mcmol/L) of population controls. |

PLACŪ Test (Lp-PLA2)

The PLACŪ test
aids in predicting risk for coronary heart disease, and ischemic stroke
associated with atherosclerosis. The PLACŪ test measures Lp-PLA2, an
enzyme that promotes inflammation inside the arteries, leading to the
formation of unstable, rupture-prone plaque. The PLACŪ test for Lp-PLA2
is different from both cholesterol tests and other tests for
inflammation because Lp-PLA2 is a specific cardiovascular risk factor
that provides unique information about the stability of the plaque
inside your arteries. Elevated levels of Lp-PLA2 increase your risk for
heart disease and stroke since plaque rupture is the major cause of both
events. |
